Cataract surgery

Cataracts are caused by proteins that can change or break down inside your eye. When this happens, the proteins can clump together and form whitish, yellow or brown clumps that cover the natural lens of your eye. These clumps impair your vision by making things blurry or hazy.

Cataracts take years to develop and are more common in older people.

Cataract surgery usually takes place in an operating room. It is generally a quick and safe procedure with a high success rate.

You will usually receive local anesthesia during cataract surgery. This numbs the nerves around your eye so you won’t feel any pain or discomfort.

During the procedure, a doctor will remove the cloudy natural lens from your eye. They will replace it with a transparent artificial lens implant called an intraocular lens.

Blurred vision is common immediately after cataract surgery. It usually disappears in 2 to 3 days. In some cases, however, it may take a week or more for the blurred vision to go away.

In addition to blurry vision, it is not uncommon to experience some sensitivity to light immediately after surgery. More serious complications are rare.

Vitrectomy

A vitrectomy is eye surgery that removes the vitreous humor from your eye. This is a gel-like substance that fills the back chamber of your eye.

You may need to remove the vitreous if you have problems with your retina. The retina is a collection of cells at the back of your eye that sends visual signals to your brain to help it interpret images.

A vitrectomy is done in an operating room. You will usually receive a longer-acting local anesthetic around your eye.

After a surgeon removes the vitreous, they replace it with a saline solution or bubble. Over time, your body will produce more natural fluid, refilling the eye socket.

If there are no complications, most people leave the hospital within hours of having a vitrectomy.

Blurred vision after vitrectomy is possible and can last 2-3 days. However, blurred vision may last longer if you have both vitrectomy and retinal surgery.

After the procedure, some people also experience a gritty or scratchy sensation in their eyes. Both issues should disappear within a few days. The surgeon may prescribe eye drops to help with healing.

Eyelid surgery

Blepharoplasty is an eyelid surgery that corrects droopy eyelids.

Typically, you will receive local anesthesia instead of general anesthesia. This means that recovery after eyelid surgery is relatively quick.

During this procedure, a surgeon will remove fat, muscle, or excess skin from your eyelid. They will then stitch up the eyelid to prevent it from falling.

LASIK or PRK surgery

Laser-assisted in situ keratomileusis (LASIK) is a common eye surgery used to treat nearsightedness, farsightedness, and astigmatism.

As with many eye surgeries, local anesthesia is commonly used and you can go home within hours.

These could be signs of glaucoma, the second most common cause of preventable blindness worldwide. “It is a disease that damages the optic nerve of the the eyes, a nerve that connects to the brain, allowing us to see. It may be related to increased pressure in the eye known as intraocular pressure,” said Dr. Neha Chaturvedi, Consultant Ophthalmologist, ASG Eye Hospital Private Limited, Bhopal. Every year, March 12 is celebrated as World Glaucoma Day. Here’s what to know about the condition that’s also known as the “silent thief of sight.”

Experts added that the prevalence of the disease increases with age and people over 40 are more susceptible. “It is estimated that in India there are about 11 million glaucoma patients over the age of 40. Although glaucoma is more common in older people, there are also types that belong to different age groups. The disease present in newborns is called “congenital glaucoma”, in young people (3-10 years old) it is called “developmental glaucoma” and “juvenile glaucoma” is called the group between 10 and 40 years old. ,” she continued.

Because the anatomical structure of each eye is different, there are two main types of glaucoma: open-angle glaucoma (OAG), which has a slow and asymptomatic onset, and angle-closure glaucoma (ACG), which causes an acute increase eye pressure.

Although there is currently no cure available for glaucoma, the eye disorder can be managed through various therapies to prevent further loss of vision and field in the patient. There are different options for lowering the eye pressure at the desired level. In its early stages, the disease can be controlled by prescribing eye drops to be used regularly. However, for cases that do not respond to medical treatment, eye surgery is an option that can slow or stop the progression of the disease. Surgeries like standard filter surgery with the use of an ologen implant, valve surgery, minimally invasive glaucoma surgery (MIGS), laser surgery are the various treatment options that can be explored with the doctor.

Successfully treating glaucoma is a team effort between patient and doctor. After prescribing a desired treatment, an ophthalmologist will lay down certain instructions that should be followed consistently. “Regular screening, annual or semi-annual comprehensive eye exams, maintaining a healthy body weight by eating a balanced diet rich in antioxidant foods, controlling diabetes, preventing high caffeine intake and smoking are some of the remedies precaution to help combat the disorder. In addition, one should also avoid head-down postures in some forms of exercise and sudden sips of large amounts of water which can cause an increase in eye pressure,” she said.

Yearly screening, regular checkup with the eye doctor, taking the prescribed medication for life, are some of the ways to manage the eye disorder and prevent vision loss. Following these key elements will help the patient lead a healthy life. In addition, awareness is the key to controlling glaucoma.

Neuro-optometry is a rapidly expanding field, with more and more optometrists becoming more involved in the diagnosis and management of certain serious and common neurological diseases. Pilar Vergara, DOO, FCOVD, who practices in Albacete, Spain, Richard Castillo, OD, DO, of Tahlequah, OK, and Kelsey Moody Mileski, OD, FAAO, of Marietta Eye Clinic, Marietta, GA, provided their take on these diseases.

Amblyopia: a paradigm shift

Pilar Vergara, DOO, FCOVD, described a new treatment paradigm for the management of anisometropic amblyopia. She and her colleague Robert Sanet, OD, developed a new protocol to circumvent the shortcomings of the current standard treatment, i.e. the patch.

These disadvantages include resistance to wearing the patch, difficulty in school and play due to the need to use the amblyopic eye to process information and perform, disappointing results with long treatment, effects long-term psychological effects on children and parents resulting from the patch and difficulties achieving 20/20 vision and maximum stereopsis, Dr. Vergara said.

Unlike the old treatment protocol, which states that the primary problem is reduced visual acuity, the patch is the optimal treatment, and the optimal prescription is based on greatest to best visual acuity, Drs. Sanet and Vergara proposed a new neuroscience-based treatment for amblyopia.

And, indeed, they believe that reduced vision is not the problem but only a symptom of the underlying binocular problem.

This thinking represents a paradigm shift.

They described their keys to success as follows: the timing and sequence of each step is crucial; the prescription of lenses is based on obtaining maximum binocularity and not maximum visual acuity; unequal additions are frequently prescribed; monocular fixation in a binocular field; optometric phototherapy; use of graduated occlusion; and optometric vision therapy.

In less complicated cases, i.e. defined as best corrected visual acuity (BCVA) of 20/50 or better and anisometropia less than 6 diopters (D), using the new protocol, 80% of patients can achieve 20/20 vision and 20 arc seconds of stereopsis without the need for a patch over 12 to 15 weeks.

In more complicated cases, defined as 20/50 to 20/200 BCVA and anisometropia up to 8 D or greater, most patients can achieve 20/20 to 20/25 vision and 20 to 30 seconds of stereopsis without any patch within a maximum period of 24 weeks.

Drs. Sanet and Vergara can explain the neuroscience research behind their new model and the steps needed to achieve better clinical outcomes. They have designed 2 courses that are currently available: a new amblyopia treatment paradigm and tearless amblyopia treatment – faster and better results without a patch.

Thyroid eye disease (TED): it takes a whole village

Dr. Castillo described current knowledge of PDD, which most commonly manifests as Graves’ disease (hyperthyroidism, however, can also develop in hypothyroid and euthyroid patients (normal hormone levels). The disease is complex and patients can benefit from the expertise of a number of specialists and sub-specialists.

The complexity of this disease is highlighted by the many attempts to classify its many manifestations. There are currently no less than 5 classification schemes. Type I includes cases with minimal inflammation and restrictive myopathy, and type II moderate to severe inflammation and restrictive myopathy.

NO-SPECS includes 7 classes of PDD ranging from Class 0 with no signs of symptoms to Class 6 with vision loss and optic nerve involvement.

The VISA classification, i.e. vision, inflammation, strabismus and appearance, defines vision, inflammation and congestion based on documented changes in inflammation rather than absolute value , strabismus and motility with measured ductions and alignments, and appearance and exposure. A score of 5 or greater when using this classification indicates active disease or progression for which steroids should be considered.

The EUGODO classification defines mild disease as swelling and retraction of the eyelids and proptosis; moderate to severe disease as active disease that includes diplopia, proptosis greater than 25 mm, and extraocular muscle dysfunction.

The classic clinical features of thyroid (Grave’s variant or hyperthyroid) eye disease include goiter, proptosis, and pretibial myxoedema, Dr. Castillo explained.

Risk factors for developing ocular manifestations include age at onset, duration of hyperthyroidism, and smoking. These manifestations include eyelid retraction/proptosis, conjunctival chemosis/hyperemia, periorbital edema, diplopia, keratitis/exposure keratopathy, and optic neuropathy compression.

Examination may reveal eyelid signs that include fixed appearance, eyelid shift on downward gaze, eyelid retraction, incomplete and infrequent blinking, lower eyelid edema, and eyelid shift lower on upward gaze.

The standard screening tests for Graves’ disease are thyroid-stimulating hormone and free T4. Conventional treatments include beta-blockers, thioamide, radioactive iodine ablation; surgical approaches are thyroidectomy and TED-specific surgeries, ie orbital decompression, strabismus, eyelid lengthening, blepharoplasty; and more recently teprotumumab infusion (Tepezza, Horizon Therapeutics).

Clinicians involved in treatment include endocrinologists or internists, oculoplastic surgeons, strabismus surgeons, and neuro-ophthalmologists.

“Thyroid eye disease, whether due to the hyperthyroid, hypothyroid, or euthyroid state, can be considered an immunological derangement, and understanding the immunopathology involved helps explain the ocular manifestations of PDD,” said the Dr. Castillo.

Papilloedema vs pseudopapillary edema: differentiating the two

Papilledema is a medical emergency and patients should be referred to the emergency room, according to Dr. Mileski.

The disease is characterized by swelling of the optic disc secondary to elevated intracranial pressure (ICP) which is bilateral in most cases. Elevated ICP causes headache, nausea, vomiting, diplopia, pulsatile tinnitus, and transient visual obscuration.

Papilledema can result from the presence of an intracranial mass, cerebral hemorrhage, meningitis, hydrocephalus, spinal cord injury, venous drainage obstruction, and hypertension idiopathic intracranial (IIH). The presentations of these are similar, but all should be excluded from the differential diagnosis before IIH is diagnosed, she advised.

Patients with IIH have normal vision and color vision, possibly diplopia, enlarged blind spot, and elevated retinal nerve fiber layer and neuroretinal rim on optical coherence tomography (OCT) of the optic nerve. On dilated examination, the optic nerve appears raised and swollen. Darkening of the vessels, Paton’s lines and hemorrhages may be present.

In contrast, pseudopapillary edema is characterized by the absence of an optic disc cup, small disc, vascular trifurcation, drusen, and spontaneous venous pulsation.

OCT can be useful in differentiating between the two diseases by visualizing drusen and peripapillary wrinkles. Fundus autofluorescence (FAF) may also show buried optic disc drusen.

However, the future is bright for diagnosing both conditions through the use of deep learning systems.

The treatment of papilledema depends on its etiology, i.e. a mass or Chiari malformation is treated surgically, cerebral vein thrombosis by treating the underlying cause with anticoagulants, and IIH with diet, weight loss and pharmacological or surgical treatment.

A new observation about papilledema resulting from IIH is that findings suggestive of IIH on magnetic resonance imaging (MRI) may be the same as in those without IIH. Another new observation is that the prevalence rate of papilledema increased from 2.8% in patients with at least 1 sign on MRI to up to 40% when more signs of IIH were present at imagery.[MK1] . Despite the MRI results, the only way to truly diagnose IIH is through a lumbar puncture.

It is also important to remember that anemia can make IIH papilledema worse. In addition to changing diet and weight loss, the underlying anemia must also be treated. The typical treatment for papilledema is acetazolamide (500 mg twice daily and titrated up to 2 grams daily). Once the papilledema has improved, this dosage can be reduced. Optic nerve sheath fenestration may be reserved for severe cases of vision loss.

Dr. Mileski’s clinical pearls include the following:

  • Papilledema is a medical emergency
  • Symptoms, clinical examinations, OCT, and visual fields should all be used to differentiate papilledema from pseudopapillary edema

• Physicians should perform an urgent outpatient workup in case of uncertainty.

• Artificial intelligence could be useful in the future.

  • IIH is a diagnosis that requires normal MRI, magnetic resonance venography and lumbar puncture
  • IIH without papilledema is possible but rare
  • Patients with IIH should be asked about anemia

“Clinical tools can help differentiate papilledema from pseudopapilledema. If papilledema is suspected, these patients should be referred to the emergency department for appropriate diagnosis and treatment. Patients should have normal neuroimaging ( MRI, MRV) and high burst pressure for a diagnosis of IIH. These patients should also be tested for anemia as this can worsen their condition,” Dr. Mileski said.

According to the Centers for Disease Control, of the 29.1 million people with diabetes in the United States, 27.8% are undiagnosed. Another 86 million Americans have prediabetes, which can be reversed with early intervention.

“Some of them may come to us with blurred vision; other times there are no symptoms. But we can see those tale shifts in the eyes,” Steinemann said.

If left untreated, the high blood glucose levels associated with diabetes can damage the blood vessels in the eye and lead to visual impairment and even blindness. Diabetes also wreaks havoc on other organs, putting diabetics at a much higher risk of cardiovascular and kidney disease and premature death.

Meanwhile, the CDC estimates that one in three American adults with high blood pressure, or hypertension, don’t know they have it. Uncontrolled high blood pressure increases the risk of heart disease, stroke and dementia. As with diabetes and prediabetes, knowing you have high blood pressure — often called the “silent killer” — is the first step in fighting its progression. An eye exam can reveal undiagnosed hypertension by identifying thickening of the blood vessels, as well as other signs of damage.

Steinemann predicts that vision screenings will become increasingly critical as we uncover links between eye health and overall well-being — some of which are already coming to light.

A substantial 2021 study, for example, found that people with two or more systemic diseases (such as diabetes, cardiovascular disease, and/or depression) in addition to an ophthalmic condition (macular degeneration, cataracts, and/or or diabetic eye disease) were at higher risk of dementia. The increased risk was most pronounced in participants with diabetes-related eye disease, whose risk of dementia was 61% higher than participants who did not have an ophthalmic problem.

“We’re also learning that cataract surgery can ward off dementia,” Steinemann said. “A very interesting study that was just published last month that took place at the University of Washington and followed over 3,000 patients for eight years found that patients who had cataract surgery had a reduced risk of dementia by 30%.

Spyros Papadatos enjoys getting back to doing what he loves.

The Sydney teenager joined his local surf lifesaving club three years ago, before COVID-19 hit.

Since then, several lockdowns have prevented him from swimming as often as he would have liked.

And they also had an unexpected impact.

“Now I have to wear sunglasses on the beach because I can’t see as well,” he said.

“If you want to help monitor people [on patrol]you need to be able to see if they have any issues.

“If you can’t see, it’s kind of hard to do it.”

COVID-19 lockdowns and remote learning have resulted in a documented increase in mental health issues.

But there are other physical health impacts – like nearsightedness or nearsightedness – that are just beginning.

Optometry Australia says its members are reporting more children being diagnosed with nearsightedness and worsening vision in some children who are already nearsighted.

This confirms international research which found, in a cohort of more than 100,000 children, the rate of myopia had more than doubled since the start of the pandemic.

Spyros was first diagnosed with mild myopia when he was nine years old.

When he returned to class after the closings, his vision loss was glaring.

“I was sitting in the back of the room and I just couldn’t see like I used to,” he said.

Increased time spent indoors leading to vision problems and injuries

According to Spyros optometrist Kay Koutzas, he is just one of many children and teenagers who have experienced deterioration in vision over the past two years.

“We certainly saw an increase in the number of children with myopia and progression of myopia,” she said.

“The combination of increased time indoors, along with long periods of work with few breaks, really drove this spike, as it puts a lot of stress on the eyes.

“This stress leads to the elongation of the eyeball. The result of this, then, is having blurred vision in distance myopia.”

Ms Koutzas says eye tests can detect early signs of myopia.

“A third of Australian children have never had an exam. And that’s really concerning.”

She says getting outdoors often and taking regular breaks from screens is also essential to help reduce the risk of developing the condition.

Muscle injuries also on the rise

Physiotherapists have also noticed an increase in injuries among children and teens returning to exercise after lockdowns.

“We had a lot more presentations of neck pain, back pain and even joint problems and headaches,” said Paul Visentini, a Melbourne-based sports physiotherapist.

One of his clients, teenager Harriet Hudson, tried to stay active during Melbourne’s extended lockdowns. But even she got injured when she started participating in team sports again.

“I missed a bit of sport because of [a knee injury],” she says.

“Then I suffered another quad injury, which really put off my comeback even more.

“It was quite frustrating to see everyone on the pitch as I sat in bed for the last two months and then had to sit on the bench again watching everyone play.”

Mr Visentini said his clinic had seen a “huge spike” in injuries after each lockdown, as children returned to sport after a long period of inactivity.

But he said most children would make a full recovery.

‘Interesting trends’ in baby development in NSW and Victoria

Researchers from the University of Western Sydney followed Australian babies born during the pandemic to find out what the impact has been on their development.

Rima Rattray, 34, gave birth to her twin daughters during an instant lockdown in Melbourne in February 2021.

Her daughters Mia and Ava were born four weeks premature and subsequent closures meant she had little interstate family support.

“Throughout the year, I haven’t received any help,” she said.

“I didn’t have a chance to say, ‘Would you mind coming over so I could take a nap or take a shower or a long shower? It was really difficult.”

Sometimes she worried that they didn’t have enough interaction with others, but overall she was relaxed about their development in the first year.

“A lot of moms compare, ‘This baby is crawling, my baby isn’t crawling’ – we were lucky we didn’t have time to compare,” she said.

Rima is confident that being born during the pandemic will not affect the development of her babies in the future.

Still, experts say it’s important that the indirect health issues for children that have arisen from the pandemic remain on the agenda.

“Let’s learn the lessons of this pandemic and not waste them,” Professor Dahlen said.
