Cataract surgery

Cataracts are caused by proteins that can change or break down inside your eye. When this happens, the proteins can clump together and form whitish, yellow or brown clumps that cover the natural lens of your eye. These clumps impair your vision by making things blurry or hazy.

Cataracts take years to develop and are more common in older people.

Cataract surgery usually takes place in an operating room. It is generally a quick and safe procedure with a high success rate.

You will usually receive local anesthesia during cataract surgery. This numbs the nerves around your eye so you won’t feel any pain or discomfort.

During the procedure, a doctor will remove the cloudy natural lens from your eye. They will replace it with a transparent artificial lens implant called an intraocular lens.

Blurred vision is common immediately after cataract surgery. It usually disappears in 2 to 3 days. In some cases, however, it may take a week or more for the blurred vision to go away.

In addition to blurry vision, it is not uncommon to experience some sensitivity to light immediately after surgery. More serious complications are rare.

Vitrectomy

A vitrectomy is eye surgery that removes the vitreous humor from your eye. This is a gel-like substance that fills the back chamber of your eye.

If there are no complications, most people leave the hospital within hours of having a vitrectomy.

Blurred vision after vitrectomy is possible and can last 2-3 days. However, blurred vision may last longer if you have both vitrectomy and retinal surgery.

After the procedure, some people also experience a gritty or scratchy sensation in their eyes. Both issues should disappear within a few days. The surgeon may prescribe eye drops to help with healing.

Eyelid surgery

Blepharoplasty is an eyelid surgery that corrects droopy eyelids.

Typically, you will receive local anesthesia instead of general anesthesia. This means that recovery after eyelid surgery is relatively quick.

During this procedure, a surgeon will remove fat, muscle, or excess skin from your eyelid. They will then stitch up the eyelid to prevent it from falling.

You will likely go home a few hours after surgery. However, short-term side effects like blurred vision and light sensitivity are possible. These usually disappear within a few days.

LASIK or PRK surgery

Laser-assisted in situ keratomileusis (LASIK) is a common eye surgery used to treat nearsightedness, farsightedness, and astigmatism.

As with many eye surgeries, local anesthesia is commonly used and you can go home within hours.

During the operation, a surgeon will use lasers and small blades to reshape the cornea of ​​your eye. This will allow your eyes to properly focus the light and should result in better vision. It generally eliminates the need for corrective lenses like glasses or contacts.

The FDA has approved a new prescription eye drop that temporarily lifts droopy eyelids.

Droopy eyelids, medically called “blepharoptosis,” can make you look tired.

While a surgical eyelid lift has been the most common treatment for droopy eyelids, now there’s a new non-surgical option called Upneeq.® It’s the first FDA-approved prescription eye drop that temporarily lifts the eyelids drooping.

How does Upneeq work?

Upneeq is a solution of oxymetazoline hydrochloride which causes the muscle behind the eyelid, called Müller’s muscle, to contract. This lifts the upper eyelid, which gives a more open appearance.

This is the same active ingredient and mechanism used in nasal sprays, which use oxymetazoline to cause blood vessels in the nose to constrict, opening the nasal cavity to facilitate breathing.

How long does Upneeq last?

Upneeq is a once-daily eye drop that has been shown to last for at least six hours in clinical trials.

What are the side effects?

Mild side effects including redness, inflammation, dry eyes, and temporarily blurred vision have been reported with Upneeq. If you experience any of these or other side effects, let your healthcare provider know so they can determine if Upneeq is right for you.

Who is a good candidate for Upneeq?

Men or women with mild to moderate droopy eyelids may be good candidates for Upneeq. For more severe cases of blepharoptosis, especially when vision is impaired, surgery may still be the best option.

Upneeq may interfere with certain medications, including beta-blockers, blood pressure medications, and others. Always tell your doctor about all medications and supplements you are taking before starting any new prescription treatment.

However, Upneeq is not for everyone. One of my young nurses who already has big, beautiful eyes tried it on and it looked like a bug for about six hours. However, it is safe and temporary and costs around $200 for a bottle.

At the end of the line

Droopy eyelids sometimes occur temporarily after receiving Botox injections. Upneeq is a new easy non-surgical treatment for droopy eyelids. If you want your eyes to look more open, talk to your doctor to see if this prescription eye drop might be right for you.

Diabetes and high blood pressure are among the most common hidden health conditions detected during eye exams. Steinemann said diabetes is of particular concern because of the growing death toll and the potential for significant vision loss and other serious complications.

According to the Centers for Disease Control, of the 29.1 million people with diabetes in the United States, 27.8% are undiagnosed. Another 86 million Americans have prediabetes, which can be reversed with early intervention.

“Some of them may come to us with blurred vision; other times there are no symptoms. But we can see those tale shifts in the eyes,” Steinemann said.

If left untreated, the high blood glucose levels associated with diabetes can damage the blood vessels in the eye and lead to visual impairment and even blindness. Diabetes also wreaks havoc on other organs, putting diabetics at a much higher risk of cardiovascular and kidney disease and premature death.

Meanwhile, the CDC estimates that one in three American adults with high blood pressure, or hypertension, don’t know they have it. Uncontrolled high blood pressure increases the risk of heart disease, stroke and dementia. As with diabetes and prediabetes, knowing you have high blood pressure — often called the “silent killer” — is the first step in fighting its progression. An eye exam can reveal undiagnosed hypertension by identifying thickening of the blood vessels, as well as other signs of damage.

Steinemann predicts that vision screenings will become increasingly critical as we uncover links between eye health and overall well-being — some of which are already coming to light.

A substantial 2021 study, for example, found that people with two or more systemic diseases (such as diabetes, cardiovascular disease, and/or depression) in addition to an ophthalmic condition (macular degeneration, cataracts, and/or or diabetic eye disease) were at higher risk of dementia. The increased risk was most pronounced in participants with diabetes-related eye disease, whose risk of dementia was 61% higher than participants who did not have an ophthalmic problem.

“We’re also learning that cataract surgery can ward off dementia,” Steinemann said. “A very interesting study that was just published last month that took place at the University of Washington and followed over 3,000 patients for eight years found that patients who had cataract surgery had a reduced risk of dementia by 30%.

The Sydney teenager joined his local surf lifesaving club three years ago, before COVID-19 hit.

Since then, several lockdowns have prevented him from swimming as often as he would have liked.

And they also had an unexpected impact.

“Now I have to wear sunglasses on the beach because I can’t see as well,” he said.

“If you want to help monitor people [on patrol]you need to be able to see if they have any issues.

“If you can’t see, it’s kind of hard to do it.”

COVID-19 lockdowns and remote learning have resulted in a documented increase in mental health issues.

But there are other physical health impacts – like nearsightedness or nearsightedness – that are just beginning.

Optometry Australia says its members are reporting more children being diagnosed with nearsightedness and worsening vision in some children who are already nearsighted.

This confirms international research which found, in a cohort of more than 100,000 children, the rate of myopia had more than doubled since the start of the pandemic.

Spyros was first diagnosed with mild myopia when he was nine years old.

When he returned to class after the closings, his vision loss was glaring.

“I was sitting in the back of the room and I just couldn’t see like I used to,” he said.

Increased time spent indoors leading to vision problems and injuries

According to Spyros optometrist Kay Koutzas, he is just one of many children and teenagers who have experienced deterioration in vision over the past two years.

“We certainly saw an increase in the number of children with myopia and progression of myopia,” she said.

“The combination of increased time indoors, along with long periods of work with few breaks, really drove this spike, as it puts a lot of stress on the eyes.

“This stress leads to the elongation of the eyeball. The result of this, then, is having blurred vision in distance myopia.”

Ms Koutzas says eye tests can detect early signs of myopia.

“A third of Australian children have never had an exam. And that’s really concerning.”

She says getting outdoors often and taking regular breaks from screens is also essential to help reduce the risk of developing the condition.

Muscle injuries also on the rise

Physiotherapists have also noticed an increase in injuries among children and teens returning to exercise after lockdowns.

“We had a lot more presentations of neck pain, back pain and even joint problems and headaches,” said Paul Visentini, a Melbourne-based sports physiotherapist.

One of his clients, teenager Harriet Hudson, tried to stay active during Melbourne’s extended lockdowns. But even she got injured when she started participating in team sports again.

“I missed a bit of sport because of [a knee injury],” she says.

“Then I suffered another quad injury, which really put off my comeback even more.

“It was quite frustrating to see everyone on the pitch as I sat in bed for the last two months and then had to sit on the bench again watching everyone play.”

Mr Visentini said his clinic had seen a “huge spike” in injuries after each lockdown, as children returned to sport after a long period of inactivity.

But he said most children would make a full recovery.

‘Interesting trends’ in baby development in NSW and Victoria

It is still unclear how the pandemic has affected our youngest children.

Researchers from the University of Western Sydney followed Australian babies born during the pandemic to find out what the impact has been on their development.

Lead researcher Hannah Dahlen says the longitudinal study involves about 6,000 families from across the country.

“We look at things like fine motor skills, more gross motor skills, how a baby communicates, interacts with how easily they settle down, how they take things in with their fingers, for example, their temperament,” said said Professor Dahlen.

Early findings suggest that some six-month-olds born in states with lockdowns and high rates of COVID-19 were slightly delayed in reaching their developmental milestones.

“In New South Wales and Victoria we are seeing some interesting trends in baby outcomes,” she said.

“At the moment, it looks like the prolonged stress is having some impact.”

Prof Dahlen said it was important to document the impact of the pandemic on Australian babies so the trajectory could be changed for those struggling.

And for babies who are overdue, there is promising news. Professor Dahlen says that while some studies abroad show babies with developmental delays at six months, they can often catch up to their peers by the time they are one year old.

“Six months is very early. You can see things at six months, as some international studies have done, that are no longer apparent at 12 months,” she said.

The study also examines the mother’s stress level and its impact on the development of her baby.

Professor Dahlen says how the mother interacts with her baby during these first weeks and months of life is key.

“It’s when language is formed, it’s when all of our social landmarks and understandings are formed. Faces are really, really important. Words are really important,” she said.

“So the tone of a mother’s voice, the amount of words spoken by the mother, the interaction, the joy, the connection with that mother are going to be potentially shaped if that mother is distressed and isolated.”

Rima Rattray, 34, gave birth to her twin daughters during an instant lockdown in Melbourne in February 2021.

Her daughters Mia and Ava were born four weeks premature and subsequent closures meant she had little interstate family support.

“Throughout the year, I haven’t received any help,” she said.

“I didn’t have a chance to say, ‘Would you mind coming over so I could take a nap or take a shower or a long shower? It was really difficult.”

Sometimes she worried that they didn’t have enough interaction with others, but overall she was relaxed about their development in the first year.

“A lot of moms compare, ‘This baby is crawling, my baby isn’t crawling’ – we were lucky we didn’t have time to compare,” she said.

Rima is confident that being born during the pandemic will not affect the development of her babies in the future.

Still, experts say it’s important that the indirect health issues for children that have arisen from the pandemic remain on the agenda.

“Let’s learn the lessons of this pandemic and not waste them,” Professor Dahlen said.

“Let’s learn what worked and what didn’t and take them into the future. This is unlikely to be our last pandemic.”

I was irritated to read a charity report last week which seemed to suggest that GPs were ‘ignoring’ the signs of ovarian cancer.

This, along with the inability of patients to detect symptoms on their own, is thought to be responsible for thousands of unnecessary deaths each year due to late diagnosis.

But the story is not so simple.

It’s true that the telltale signs of ovarian cancer – bloating and pain in the lower abdomen – can easily be confused with more common and less serious conditions such as irritable bowel syndrome.

The truth is that not all doctors can refer patients for all the tests we think they need due to a lack of resources in some areas.

In my North London practice I can easily refer patients for gynecological ultrasounds, but I know a colleague in another part of the country who can only do so if the patient is high risk.

Instead of blaming it all on GPs, charities should lobby health chiefs to stamp out the postcode lottery within our NHS.

Ministers should identify regions with limited resources and invest more money and personnel in the system.

We know all too well the signs of ovarian cancer, but if we can’t order a test, early diagnosis is next to impossible.

The COVID-19 pandemic has led to an increased use of digital screens as well as an increase in e-learning days which had already been implemented in our schools.

Unfortunately, there can be significant downsides to spending so much time in front of a screen.

Although the long-term implications for current school children are unclear, eye care professionals have expressed some concern about a problem called convergence insufficiency, which sometimes persists for a lifetime.

Convergence insufficiency is an alteration of the proper functioning of the eyes. Symptoms include headaches when reading, words that seem to move across the page, blurring, double vision, and eye strain. It can be treated with exercise, prism glasses, or (rarely) surgery.

Previous research has linked the use of digital screens to convergence insufficiency. So when many schools switched to distance learning for the pandemic, researchers at Wills Eye Hospital in Philadelphia studied whether it would impact students’ eyes.

They surveyed 110 healthy students aged 10 to 17 about eye symptoms before and after a virtual school day. Participants spent an average of about seven hours a day in a virtual school.

Participants completed the Convergence Insufficiency Symptoms Survey (CISS), which consists of 15 questions about eye complaints: 61% of participants reported an increase in convergence insufficiency symptoms.

The researchers also found that, on average, the more hours each student spent in a virtual school, the higher their CISS scores.

This makes sense because reading requires convergence. The same problem could occur in the traditional school if students stared at books all day instead of focusing on objects at different distances in their classrooms, such as the teacher or the whiteboard.

Parents can help prevent convergence insufficiency during school by reminding their children to take breaks. The 20/20/20 rule is recommended: After 20 minutes of work that involves looking at nearby objects, students should pause for 20 seconds and look at something 20 feet away.

For a more local perspective, I contacted optometrist Dr. Craig Lichlyter who gave the following comments:

“I wholeheartedly agree with the conclusion of the research, based both on the ‘book science’ of binocularity (eyes working as a team) and based on patients seen in the practice day in and day out! This is a problem, not only for pupils aged 10 to 17!

“Convergence insufficiency is a problem for those who, in the workplace or in the classroom, spend blocks of time exceeding two continuous hours on a PC, laptop, notebook or smartphone. The researchers put developed a quantitative assessment of the clinical symptoms that we hear daily in the office: “My eyes are very tired at midday”, “My eyes are slow to focus from far to near and vice versa”, “I am having difficulty staying focused on the computer, especially towards the end of the day Convergence insufficiency can affect all patients between the ages of 5 and 50.

“Lack of blinking when focusing on near objects, especially digital targets, is well documented in the medical literature as problematic. It causes or worsens dry eye, a medical condition known as keratitis This condition is quite uncomfortable and causes blurred vision.

“I agree that convergence insufficiency can be best addressed by taking digital demand pauses (like the 20-20-20 rule mentioned in this article), but what many do during that pause is pull out their smartphone, which does nothing to relieve eye stress!

“New eyeglass lens technologies have created a very good treatment for convergence insufficiency. These lenses are generated with advanced digital technology using computer controlled diamond blades. The lenses, called Accommodative Support Lenses, provide a “reading boost” at the bottom of the lens, which is different from the prescription needed for distance vision. It helps the eye focus on your device (PC, Laptop, Notebook, Smartphone) and helps relieve eye strain. The lens is not a bifocal lens, and it is not a progressive addition lens.

“Researchers did not specify another concern with increased use of the devices, which is exposure to visible blue light. Accommodatively-supported lenses are treated with a high-quality blue blocker (all blue blockers blue are not of good quality), which reduces the visible blue light reaching the eye.

“We are proactively prescribing accommodating supportive lenses with a blue light blocker to all patients who use a screen for a significant portion of their day. We hear daily from these patients that their lenses have relieved eye fatigue and improved comfort at the end of their day.
